Remote Care Coordinator for Virtual Patient Care

Lara Health is seeking a compassionate and experienced Care Coordinator to join our virtual care team. This is a fully remote role with flexible scheduling, ideal for someone who is confident speaking with patients, explaining healthcare programs clearly, and helping patients get started with services that can improve their health and access to care.


The Care Coordinator plays an important front-line role in introducing patients to new care programs, explaining their benefits, answering questions, and supporting enrollment and onboarding.


Position Highlights

Schedule: Minimum 35 hours/week

Location: Fully remote (U.S.-based only)

Experience Required: Minimum 3–5 years in patient-facing healthcare support, care coordination, medical office, or chronic care settings

Pay: $22-30/hour plus bonuses, total compensation based on prior experience and performance


Role Responsibilities

  • Contact eligible patients by phone to introduce and explain new care programs

  • Educate patients on the benefits of programs such as chronic care management, remote monitoring, and other preventive care services

  • Answer patient questions clearly and professionally, helping them understand how the programs work

  • Guide patients through the onboarding and enrollment process

  • Document patient interactions accurately in EMR and internal systems

  • Coordinate follow-up communication as needed to support enrollment and activation

  • Work closely with providers, clinic staff, and internal care teams

  • Help ensure patients feel supported, informed, and confident in participating in their care

  • Perform other duties as assigned


Required Skills & Qualifications

  • Experience: 3–5+ years in care coordination, patient outreach, medical assisting, healthcare customer support, or related healthcare role

  • Language: Bilingual English/Spanish strongly preferred (but not mandatory)

  • Excellent phone presence and strong patient communication skills

  • Ability to explain healthcare services in a simple, reassuring, and engaging way

  • Strong organizational skills and attention to detail

  • Ability to work independently in a remote environment

  • Comfortable using EMR systems, online tools, and computers efficiently

  • Must have own computer and reliable internet connection

  • Familiarity with medical terminology and patient communication best practices

  • Passion for helping patients and improving access to care


Preferred Experience

  • Experience with CCM, RPM, PCM, or other virtual care programs

  • Experience onboarding patients into healthcare programs or services

  • Familiarity with remote care coordination workflows and EMR-based task management

  • Experience working with older adults and patients with chronic conditions

  • Certification in Medical Assisting, LPN, Case/Care Management or similar
