Telehealth Sales Representative

Hiring for Field Territories: Sacramento County, Solano County, SF Bay Area, San Jose - Northern California and Southern California 

We are seeking a dynamic, driven Telehealth Sales Representative to accelerate adoption of our advanced virtual care platform. Our solutions include Chronic Care Management (CCM), Remote Patient Monitoring (RPM) and Transitional Care Management (TCM) services

Emerge Health Connect is empowering healthcare providers, clinics, and health systems to strengthen care continuity, boost patient engagement, and maximize CMS reimbursements through value‑based care programs.

This role blends healthcare expertise with consultative sales strategy, requiring someone who can clearly articulate both the clinical and financial impact of telehealth, CCM, RPM and TCM solutions.

Responsibilities

Identify, prospect, and qualify healthcare providers and organizations for telehealth, CCM, RPM and TCM services.

Educate clients on the value of Virtual Care Coordination and how CCM/RPM/TCM programs enhance patient outcomes while generating new revenue streams.

Deliver tailored product presentations and live demos highlighting workflow integration, compliance advantages, and operational efficiency.

Manage the full sales cycle—from lead generation to proposals, negotiations, and closing.

Collaborate with marketing and product teams to align sales strategies with provider needs and market trends.

Serve as a trusted advisor to practices transitioning to value‑based care models.

Consistently achieve or exceed sales quotas and performance targets.

Skills & Qualifications

Proven success in healthcare sales, ideally in telehealth, population health, home health, hospice or digital health solutions.

Strong understanding of CCM, RPM, TCM, and CMS reimbursement models.

Familiarity with care coordination workflows, HIPAA compliance, and CMS regulatory requirements.

Excellent communication, consultative selling, and relationship‑building abilities.

Experience using CRM platforms and cloud‑based software tools.

Highly motivated self‑starter who thrives in a fast‑paced, independent work environment.

Pay: $26.00 - $30.00 per hour

Expected hours: 20-hour min. per week
