Chronic Care Management Enrollment Specialist – Remote

Contract Position | Hourly Base + Commission + Bonus Opportunities

We are seeking a confident, professional, and motivated Chronic Care Management (CCM) Enrollment Specialist to join our remote patient enrollment team. This is a sales-focused healthcare position handling both inbound and outbound patient calls. You will educate eligible patients about Chronic Care Management and virtual care programs, answer questions, address objections, follow up with interested patients, and guide them through enrollment.

What We’re Looking For

• Strong sales, communication, and closing skills
• Excellent, professional phone presence
• Ability to quickly build rapport and trust with patients
• Basic medical knowledge and familiarity with medical terminology
• Comfortable communicating with Medicare and older adult populations
• Fluent English required, Bilingual Spanish is an asset
• Healthcare enrollment, medical office, call center, Medicare, CCM/RPM, or telephone sales experience is an asset

Compensation: Contract position with a base hourly rate plus commission and potential performance-based bonuses.  

We’re looking for someone who can combine compassionate patient communication with a strong sales mindset—someone who listens, explains value clearly, confidently handles objections, and knows how to ask for the enrollment.
