We are privately owned CHAP Accredited Home Health Agency, based in Campbell, California. We currently provide home health care to patients in Santa Clara county, San Mateo County, San Benito county, Alameda County and Solano County, with plans to expand our service area.
Our founders include seasoned nurses, healthcare administrators and other professionals in the post-acute industry with experience in providing patient-centric care.

We are seeking a dedicated and compassionate Home Health Nurse to join our dynamic healthcare team. If you are a registered nurse with a passion for providing quality care in the comfort of patients homes, we invite you to apply for this position.
Ideally this candidate is able to cover the majority of Santa Clara County. Would be a plus if you able to cover surrounding counties on a as needed basis.
Responsibilities:
- Conduct comprehensive assessments of patients' needs and develop individualized care plans
- Attend all required case conference, staff, educations
- Complete all Oasis and or visit documentation within 24 hours of the visit date
- Collaborate with interdisciplinary teams to coordinate and facilitate patient care
- Monitor and evaluate patients' progress towards achieving their goals
- Provide education and support to patients and their families regarding treatment options and resources
- Advocate for patients' rights and ensure they receive appropriate services
- Maintain accurate and up-to-date documentation of patient interactions and interventions
Qualifications:
- Minimum Oasis Home Health experience (1+ year)
- Bachelor's degree in nursing, social work, or a related field
- Current licensure or certification in a relevant discipline (e.g., RN, LCSW)
- 2+ years experience working in a healthcare setting, preferably in emergency room, critical care or ICU- Strong assessment, communication, and problem-solving skills
- Ability to work independently and as part of a team
- Knowledge of community resources and services for patient referrals
- Proficiency in electronic medical records systems
