Compensation: Up to $77 DOE home health oasis experience
We are seeking dedicated and compassionate Home Health Nurse(s) for our Alameda County territory to join our dynamic healthcare team. If you are a RN with a passion for providing quality care in the comfort of patients’ homes, we invite you to apply for this position.
Responsibilities:- Conduct comprehensive assessments of patients' needs and develop individualized care plans- Collaborate with interdisciplinary teams to coordinate and facilitate patient care- Monitor and evaluate patients' progress towards achieving their goals- Provide education and support to patients and their families regarding treatment options and resources- Advocate for patients' rights and ensure they receive appropriate services- Maintain accurate and up-to-date documentation of patient interactions and interventions
Qualifications:- Bachelor's degree in nursing, social work, or a related field- Current licensure or certification in a relevant discipline (e.g., RN, LCSW)- Experience working in a healthcare setting, preferably in emergency room, critical care, behavioral health, or ICU- Strong assessment, communication, and problem-solving skills- Ability to work independently and as part of a team- Knowledge of community resources and services for patient referrals- Proficiency in electronic medical records systems
-Home Health experience (1+ year) REQUIRED
