LVN Scheduler and Care Coordinator - Home Health

Salary Range:  $50 - $55 hour

Ascension Healthcare Services is looking for a dynamic, detailed oriented, and experienced LVN Scheduler/Intake and Care Coordinator to join our growing team!

We are privately owned, CHAP Accredited Home Health Agency based in Campbell, California. We provide care in Santa Clara, San Mateo, San Benito county, Alameda County, and Solano County with plans to grow.

Position Summary:

We are seeking an experienced and detail-oriented LVN to oversee our Intake Dept to include Intake, Scheduling and Clinical documentation review. This role is vital in ensuring smooth onboarding of new patients, appropriate clinical documentation, and efficient coordination of care across our home health services.

Responsibilities:

  • Oversee all agency Scheduling tasks for 40+ clinical employees
  • Maintain and report all daily, weekly scheduling capacity and productivity reports for Executive Director
  • Establish and maintain outstanding relations with field staff, sales, office and leadership
  • Back up Intake Coordinator on all incoming referrals, including verifying insurance, verifying following MDs, welcome calls, collaborating with sales team
  • Review clinical Intake documentation for accuracy, completeness, and compliance with Medicare/home health guidelines
  • Assist in ensuring clinical appropriateness of services ordered and collaborate with sales on documentation requirements
  • Enter and maintain accurate admission information in patient EMR system
  • Monitor and maintain compliance of Medicare Timeliness of Initiation of Care on all incoming referrals and ensure appropriate orders, notes are entered into EMR
  • Schedule home health visits within Medicare Guidelines to ensure all patient needs are.
  • Update all shared capacity and scheduling reports for staff and leadership
  • Maintain updated calendars and schedules for all field clinical staff
  • Communicate effectively with referral sources, physicians, patients, and caregivers
  • Collaborate with clinical managers to uphold regulatory standards
  • Monitor and resolve scheduling conflicts or coverage gaps proactively

Qualifications:

  • Active LVN license in state of CA
  • Minimum of 2 year experience in Medicare Home Health. (Private Caregiving does not apply)
  • Experience managing a scheduling console for 50+ employees
  • Must have some proven administrative background
  • Experience in Intake and clinical documentation review strongly preferred
  • Familiarity with home health regulations, Medicare guidelines, and OASIS documentation
  • Excellent attention to detail and critical thinking skills
  • Strong communication and collaborative skills
  • Proficiency in EMR systems (e.g., Kinnser, WellSky, or similar)
