We are privately owned, CHAP Accredited Home Health Agency based in San Jose, California. We provide care in Santa Clara, San Mateo and San Benito counties, and are expanding services to Alameda, Contra Costa, Solano counties!
We have an immediate need in Solano County for a Per Diem Occupational Therapist.
Our founders include 150+ years of highly experienced nurses, healthcare administrators and other professionals in the post-acute industry dedicated to the highest quality clinical outcomes and patient satisfaction.

Compensation: DOE in home healthcare setting
Overview: We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will work with a diverse population, including individuals with autism, in various settings such as schools, outpatient clinics, and acute care facilities. This role involves assessing patient needs, developing individualized care plans, and implementing therapeutic interventions to enhance the quality of life for patients.
Responsibilities
- Conduct comprehensive evaluations to assess patients' functional abilities and needs.
- Develop and implement personalized treatment plans based on assessment results.
- Provide therapy services in a home environment
- Collaborate with interdisciplinary teams to ensure coordinated care and effective discharge planning.
- Educate patients and their families on therapeutic techniques and strategies for daily living.
- Maintain accurate documentation of patient progress and treatment outcomes.
- Stay updated on best practices in occupational therapy and participate in professional development opportunities.
Qualifications
- Degree in Occupational Therapy from an accredited program.
- Valid state licensure or eligibility for licensure as an Occupational Therapist.
- Knowledge of anatomy and physiology relevant to occupational therapy practice.
- Experience in case management and developing care plans is preferred.
- Familiarity with working with individuals with autism is a plus.
- Strong communication skills and the ability to work collaboratively within a team setting.
- Previous experience in school-based therapy or acute care settings is advantageous.
