We are privately owned CHAP Accredited Home Health Agency, based in San Jose, California. We currently provide home health care to patients in Santa Clara county, San Mateo County and San Benito county, and are expanding into Alameda, Contra Costa & Solano counties.

Ascension Healthcare has an immediate need for a Home Health Physical Therapist in San Benito County (Hollister, CA).  Primary market will be Morgan Hill to Hollister areas. 

Salary - DOE with Oasis Home Health 

Duties:- Evaluate and assess patients' physical conditions and develop individualized treatment plans- Provide hands-on therapy, including exercises, stretches, and manual techniques- Educate patients on proper techniques for exercises and activities to improve their condition- Monitor and document patients' progress and adjust treatment plans as necessary- Collaborate with other healthcare professionals to coordinate patient care- Stay up-to-date with the latest research and advancements in physical therapy

Experience:- Bachelor's or Master's degree in Physical Therapy- Valid state license to practice as a Physical Therapist- Strong knowledge of anatomy, physiology, and medical terminology- Experience in a hospital or acute care setting is preferred- Specialized training or experience in trigger point therapy or pediatrics is a plus

As a Physical Therapist, you will play a crucial role in helping patients recover from injuries, manage chronic conditions, and improve their overall physical well-being. You will work closely with patients to develop personalized treatment plans and provide hands-on therapy to help them regain strength, mobility, and function.

In this role, you will need a strong understanding of anatomy, physiology, and medical terminology to effectively assess patients' conditions and develop appropriate treatment plans. Experience in a hospital or acute care setting is preferred as it will allow you to work with a diverse range of patients with varying needs.

Your ability to provide compassionate patient care, excellent communication skills, and attention to detail will be essential in helping patients achieve their goals. Additionally, specialized training or experience in areas such as trigger point therapy or pediatrics can be beneficial.

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Eligible roles also qualify for a comprehensive benefits package.  $10 Sign On Bonus for experienced home healthcare candidates only.
