Billet Health is excited to announce a career opportunity for a dedicated and compassionate professional in the role of Registered Nurse, specialized in Home Health and Hospice care. This Full-Time position is geared towards those wishing to make a significant impact in the lives of patients, offering personalized home health and hospice services. Note that this opportunity is not a remote job and requires presence in the patient's or facility's location.

Our ideal candidate is a registered nurse who brings both passion and expertise in managing a diverse array of patient health scenarios through detailed, personalized treatment plans administrated over a typical 9-week care episode.
Duties and Responsibilities
- Conduct initial patient assessments and regular follow-ups to adjust care plans as needed.
- Provide direct patient care in the home or residential facility setting.
- Coordinate with healthcare team members to facilitate integrated and comprehensive care.
- Document patient care services by charting in patient and department records.
- Adhere to professional standards, home care policies and procedures, and federal, state, and local requirements.
- Educate patients and their families on symptom management and health maintenance during the treatment process.
- Modify patient treatment plans as indicated by patients' responses and conditions.
- Maintain a safe and clean working environment by complying with procedures, rules, and regulations.
- Ensure continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs.
- Use clinical expertise to plan, deliver, and evaluate personalized care for patients.
Requirements
- Active RN License in the state of practice.
- Valid Driver License and access to a dependable vehicle.
- Current CPR or BLS certification.
- Valid TB test (Skin test within the last year or X-ray within the last five years).
- Strong interpersonal and communication skills to effectively interact with patients, families, and other healthcare team members.
- Ability to work autonomously and build flexible schedules according to patient needs and coverage requirements.
- Demonstrated commitment to ongoing professional development and maintaining current knowledge of home health care techniques and practices.
- Capability to manage a varying patient caseload and willingness to travel within a designated area.
- Experience in home health or hospice care preferred, but willing to train motivated individuals new to this specialization.
