Current cases are located in Oceanside, Carlsbad, and Escondido, CA, this position guarantees hands-on experience in a varied and fulfilling environment with flexible scheduling to accommodate personal commitments. As this is an in-person role providing essential home health services, it is crucial to note that this is not a remote job. Duties and Responsibilities

  • Provide high-quality nursing care to pediatric and special needs patients with varying medical complexities in their homes.
  • Develop and update individualized patient care plans in collaboration with physicians, therapists, and families.
  • Administer medications, conduct routine check-ups, and monitor vital signs to ensure patient health and safety.
  • Assist in daily living activities and developmental tasks to enhance the quality of life for patients.
  • Counsel and educate family members on patient care, providing guidance and emotional support to empower them in the caregiving process.
  • Maintain accurate and detailed patient records, documenting all care, responses, and significant incidents to ensure continuity of care and compliance with regulations.
  • Coordinate with a multidisciplinary team to ensure a unified approach to patient care and goals.
  • Utilize advanced nursing equipment and technology effectively, maintaining operational literacy and competency.
  • Respect patient confidentiality and adhere to all professional and ethical nursing standards.
  • Communicate effectively with patients, families, and the healthcare team to facilitate optimal outcomes and patient satisfaction.

Requirements

  • Active California LVN License - we take NEW GRADS
  • Current BLS certification
  • Valid driver’s license and auto insurance
  • Interest in pediatrics and caring for clients with special needs
  • Exceptional interpersonal skills and the capability to work compassionately with patients and their families
  • Ability to work autonomously and responsibly without constant supervision
  • Excellent organizational and time management skills, with a commitment to punctuality and schedule adherence
  • Physical ability to perform job-related duties, which may require lifting, bending, stretching, and other physical activities
  • Commitment to personal and professional growth in pediatric and special needs care through continual education and development programs
