Telemed Triage MD- Emergency Med, Multi State License PCP

Position Summary

We are seeking a highly qualified Telemedicine Triage Physician to provide front-end

clinical assessment, risk stratification, and care navigation for patients through a virtual

care platform. This role is ideal for an Emergency Medicine physician or an experienced

Primary Care physician with strong urgent-care, occupational health, or acute-care

judgment. The physician will evaluate presenting concerns, determine the appropriate

level of care, identify red flags, and guide patients to the safest and most efficient next

step.

Key Responsibilities

• Provide timely virtual triage for patients presenting with acute, episodic, occupational, or general medical concerns.
• Conduct focused history-taking and video-based clinical assessments within the limits of telemedicine.
• Identify emergent or potentially high-risk conditions and direct patients to the appropriate level of care, including emergency department, urgent care, occupational medicine, primary care, or specialty follow-up.
• Apply sound clinical judgment to determine when telemedicine is appropriate and when in-person evaluation is required.
• Document encounters clearly, accurately, and in compliance with applicable clinical, legal, privacy, and payer requirements.
• Communicate clinical recommendations in a professional, patient-centered, and easy-to-understand manner.
• Collaborate with nurses, care navigators, client teams, and other clinical staff to support safe and coordinated care.
• Follow established protocols while using independent medical judgment when patient safety requires escalation. 
• Maintain current knowledge of telemedicine regulations, licensure requirements, prescribing limitations, and clinical best practices.
• Support quality assurance, case review, protocol development, and continuous clinical improvement initiatives as needed.

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.
• Active, unrestricted medical license in good standing
• Eligibility for multi-state licensure through the Interstate Medical Licensure Compact or possession of multiple active state medical licenses preferred.
• Board certification or board eligibility in Emergency Medicine, Family Medicine, Internal Medicine, or another relevant specialty.
• Minimum of 3–5 years of post-residency clinical experience, preferably in emergency medicine, urgent care, primary care, occupational medicine, or telemedicine triage. 
• Current DEA registration, where applicable to the scope of services provided.
• Current BLS certification required; ACLS certification strongly preferred for
• Emergency Medicine candidates.
• Demonstrated ability to recognize urgent, emergent, and red-flag presentations in a remote-care environment.
• Strong knowledge of evidence-based triage, care escalation, patient safety, and appropriate referral pathways.
• Excellent documentation skills and familiarity with electronic medical records or telehealth platforms.

• Board-certified Emergency Medicine physician with experience in high-volume triage, urgent evaluation, and disposition decision-making.
• Primary Care physician with broad acute-care experience and multiple active state licenses.
• Licensure in one or more compact-participating states, with willingness and ability to expand licensure based on business needs.
• Experience with occupational health, workers’ compensation, musculoskeletal complaints, return-to-work guidance, or employer-sponsored health programs.
• Prior telemedicine experience, including virtual urgent care, nurse triage oversight, or front-end clinical navigation.
• Comfort working independently in a remote environment while maintaining close communication with operational and clinical teams.
• Strong patient communication skills, including the ability to explain next steps clearly without overcomplicating medical information.

• Maintain all required state licenses, certifications, continuing medical education

• Practice only within states where the physician is appropriately licensed, registered, or otherwise authorized to provide telehealth services.
• Verify patient location and follow applicable state-specific telemedicine requirements before delivering care.
• Comply with HIPAA, privacy, documentation, informed consent, and telehealth regulatory standards.
• Adhere to organizational protocols, clinical quality standards, escalation pathways, and patient safety requirements.
• Maintain a clear, unrestricted clinical record, including no active disciplinary actions or exclusions from federal healthcare programs.

Work Environment

This is a remote telemedicine position supporting front-end triage and care navigation.

The physician must be comfortable making timely, clinically sound disposition decisions

using virtual tools and documented protocols. Availability may include scheduled shifts,

flexible coverage blocks, after-hours support, or variable demand-based coverage

depending on program needs.
