Pay Rates – Per Completed Visit

SOC / ROC - $150

Eval (complex skill add-on) - $115

Revisit / Discharge - $70 

Downtime (pre-approved wait time) - $45/hr

Premium add-ons:

• Same-Day / Urgent: +$25
• Weekend: +$25
• Holiday: +$50

"Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.

Requirements

Must-Have:

• Active PT license (Ohio) + current BLS certification
• Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
• Minimum 1 year of clinical physical therapy experience

Preferred / Nice-to-Have:

• OASIS certification or demonstrated OASIS experience
• Prior home health experience
• Familiarity with HCHB or similar home health EMR
