The position offers a part-time schedule and requires candidates to work on-site, as it is not a remote job. As a hospice aide, you will play a critical role in enhancing the quality of care our patients receive during their most vulnerable times.

Duties and Responsibilities

  • Provide direct patient care under the supervision of a registered nurse or a licensed practical nurse.
  • Assist patients with daily activities such as dressing, bathing, eating, and maintaining personal hygiene.
  • Monitor and report changes in patient health status to the supervising nurse in a timely manner.
  • Support family members with the care of the patient by educating them on proper care techniques.
  • Ensure the patient’s environment is safe and comfortable, making necessary adjustments as required.
  • Maintain accurate and up-to-date patient records in accordance with legal and organizational requirements.
  • Attend and participate in scheduled team meetings to discuss patient care and progress.
  • Provide emotional support to patients and their families, helping them to cope with the challenges of terminal illness.
  • Adhere to all privacy regulations and ensure confidentiality and security of patient health information.
  • Assist in the implementation of patient care plans developed by the healthcare team.
  • Collaborate with other healthcare professionals to enhance the provision of end-of-life care.
  • Participate in continuing education and training to improve skills and stay updated with the latest practices in hospice care.

Requirements

  • High school diploma or equivalent
  •  Licensed STNA or CNA. 
  • Minimum of two years’ experience in a hospice care or home care setting or equivalent.
  • Demonstrated ability to provide compassionate care and effectively communicate with terminally ill patients and their families.
  • Strong observational skills and ability to report changes in patient conditions accurately and promptly.
  • Knowledge of personal care and patient hygiene standards.
  • Physical ability to perform duties as outlined (e.g., lifting up to 50 lbs, providing physical support to patients).
  • Valid driver’s license and reliable transportation, as this position requires travel to patient homes within Cuyahoga County.
  • Proof of current CPR certification.
  • Exceptional interpersonal skills, with an emphasis on empathy, patience, and professionalism.
  • Capability to work effectively both independently and as part of a multi-disciplinary team.
  • Adherence to all health and safety regulations and caregiving standards.
