This part-time position offers the unique opportunity to bring expert care directly to patients' homes, providing both clinical services and compassionate support to those in need. As a PRN Home Health RN, you will integrate specialty skills, professionalism, and personalized care to help our patients maintain the highest quality of life while navigating various health challenges.
This hands-on role is suited for candidates who are proactive, self-motivated, and passionate about making a tangible difference in the lives of the patients and their families. Please note, this is an onsite, non-remote role, requiring travel within the local community to provide direct patient care.
Duties and Responsibilities
- Conduct initial patient assessments and develop individualized plans of care in collaboration with other healthcare professionals.
- Administer medications, manage pain, and deliver other necessary treatments as prescribed by the attending physician.
- Educate patients and their families on diagnosis, prognosis, and all treatment options to enable informed decisions.
- Monitor patient recovery and progress, adapting the care plan as needed, and timely documentation of the same.
- Coordinate with healthcare team members to facilitate holistic care that meets the emotional, social, and physical needs of the patient.
- Ensure compliance with all health and safety regulations during home visits.
- Provide post-operative care and support rehabilitation efforts.
- Manage emergency situations adeptly, applying critical thinking and swift decision-making skills.
- Maintain up-to-date clinical records and report on any significant changes in the patient's condition to the relevant professionals.
- Advocate for the patient’s needs and act as a liaison between patients, families, and physicians.
Requirements
- Registered Nurse (RN) license in good standing.
- Minimum of 2 years' experience in a home health or similar setting.
- Demonstrated ability to work independently and manage patient care with minimal supervision.
- Strong communication and interpersonal skills, capable of effectively coordinating with patients, families, and other healthcare professionals.
- Proficient in using healthcare software and systems for patient management and documentation.
- Ability to perform physically demanding tasks such as lifting or moving patients as needed.
- Flexibility to work on-call, evenings, weekends, or holidays according to the patient care schedules.
- Thorough understanding and sensitivity to the needs of patients from diverse backgrounds.
- Current CPR certification is required.
- Reliable transportation and valid driver’s license to visit multiple patient homes per day.
