h/care is excited to announce a job opening for a dedicated and compassionate Hospice Registered Nurse Case Manager (RNCM). As a Hospice RNCM, you will play a pivotal role in designing and implementing personalized care plans tailored to the individual needs of our patients and their families.

This full-time position is ideally suited for a candidate who thrives in a supportive and caring environment and is eager to make a significant impact in the lives of others. Please note that this is not a remote job and requires on-site attendance to provide direct patient care and collaborate with other healthcare team members.

Duties and Responsibilities

  • Conduct initial nursing assessments of new patients after referral and admission.
  • Develop and implement comprehensive nursing care plans that address the specific needs of patients.
  • Coordinate with physicians, other hospice team members, and healthcare providers to ensure the consistency and quality of care.
  • Regularly visit patients in their homes or hospice centers to monitor their condition, evaluate care effectiveness, and make necessary adjustments to the care plan.
  • Provide direct patient care, administer medications, and assist in symptom management and pain control.
  • Educate patients and their families on end-of-life care and provide emotional support to help them cope with the challenges of terminal illness.
  • Maintain accurate and detailed patient records and prepare reports as required by healthcare regulations and hospice policies.
  • Participate in interdisciplinary team meetings to discuss and evaluate patient care processes.
  • Ensure compliance with all federal, state, and local regulations as well as professional standards pertaining to hospice care practices.
  • Advocate for patient needs and serve as a liaison between patients, families, and physicians.
  • Assist in the training and supervision of junior nurses and hospice staff.

Requirements

  • Current and valid Registered Nurse (RN) license in the state of practice.
  • Minimum of two years of nursing experience, preferably in hospice or palliative care settings.
  • Demonstrated ability to develop and implement effective nursing care plans.
  • Exceptional communication and interpersonal skills, capable of handling sensitive situations with empathy and compassion.
  • Strong organizational and time-management skills, with the capacity to handle multiple priorities simultaneously.
  • Deep understanding of the hospice care philosophy and readiness to ensure that it is conveyed through all nursing procedures and protocols.
  • Ability to work independently and as part of a diverse and dedicated healthcare team.
  • Proficiency in using healthcare software and electronic health records (EHR).
  • Commitment to continuous professional development and adherence to the ethical guidelines of the nursing profession.
  • Physical and mental stamina required to meet the demands of extensive patient interaction and the emotional nature of hospice care.
