h/care, a leading provider in integrated healthcare services, is currently seeking a dedicated and compassionate professional to join our Home Health team as a PRN Home Health Medical Social Worker (MSW). This Part-Time position is an exceptional opportunity for a skilled MSW who is looking to impact the lives of patients in a home-based setting. This role offers a dynamic work environment where no two days are the same.

The selected candidate will be instrumental in offering supportive care and resources to patients recovering or living with various health conditions in their homes. As this is a community-based role, it is important to note that this position is not a remote job and requires direct patient interaction, offering tailored care plans to enhance patient quality of life and support their health recovery or maintenance.

Duties and Responsibilities

  • Conduct comprehensive assessments of patient’s social, emotional, environmental, financial, and support needs.
  • Formulate individualized care plans based on patient’s condition and medical directives.
  • Provide crisis intervention and psychological support as needed while helping patients navigate healthcare systems.
  • Act as an advocate for patients and their families to help procure services and resources in accordance with their health care needs.
  • Collaborate with healthcare providers, including physicians, nurses, and other allied health professionals, to ensure a holistic approach to health management.
  • Maintain accurate and concise documentation of patient care and interventions in compliance with applicable laws and regulations.
  • Educate patients and their families on available community resources and applicable healthcare service options.
  • Support and guide patients with advance directives, guardianship, and legal issues concerning healthcare.
  • Facilitate support groups or educational sessions for patients and family members when applicable.
  • Regularly evaluate the effectiveness of the care plan and update it as necessary.

Requirements

  • Master’s Degree in Social Work from an accredited school of social work.
  • Valid and current licensure to practice as a social worker at the master’s degree level in the residing state.
  • A minimum of two years’ experience in a health care setting, preferably in home health or similar environment.
  • Demonstrated ability to manage high caseloads and effectively prioritize tasks.
  • Strong interpersonal skills with a proven ability to communicate effectively (both written and verbal) with all levels of personnel and a diverse population.
  • Proficient understanding of chronic and acute health conditions and how they interact with mental and emotional health.
  • Knowledge of community health resources and experience in facilitating access to these resources.
  • Ability to maintain a high degree of confidentiality and adhere to HIPAA regulations.
  • Strong advocacy and negotiating skills in the interest of the patient.
  • Ability to work autonomously with limited direct supervision while maintaining a team-oriented approach.
