This role is ideal for professionals who are passionate about providing physical therapy services in a home health setting. It involves enabling patients to achieve their maximum levels of functional independence through personalized therapeutic interventions crafted in accordance with their individual conditions and goals. This position requires hands-on patient care and is not a remote job. As a part-time employee, you will play a crucial role in enriching the lives of patients, ensuring they receive the highest quality of care in their own homes.
Duties and Responsibilities
- Perform comprehensive patient assessments to establish patient-specific treatment plans focusing on functional recovery and pain management.
- Deliver high-quality, direct physical therapy according to the physician-prescribed plan of care.
- Guide and educate patients and their families on exercises and proper use of mobility aids to supplement in-home therapy sessions.
- Regularly reassess ongoing therapy and its effectiveness, adjusting treatment plans as needed to ensure optimal patient progress.
- Maintain accurate and detailed reports on patients’ therapy progress, ensuring documentation complies with company policies and regulatory requirements.
- Collaborate with other members of the healthcare team to provide an integrated approach to care, promoting overall health and wellness.
- Manage a patient caseload while ensuring attention to scheduling and follow-up visits.
- Utilize and maintain therapy equipment in a safe and effective manner, providing maintenance as required.
- Stay updated with the latest physical therapy techniques and technologies beneficial for home health care settings.
- Provide emotional support and motivation to patients throughout their treatment journey.
Requirements
- Current and valid state licensure as a Physical Therapist.
- Minimum of a Master’s degree in Physical Therapy from an accredited institution.
- Proven experience in a home health setting is preferred.
- Demonstrable knowledge of physical therapy modalities and methods applicable to the home healthcare environment.
- Excellent interpersonal skills to establish rapport with patients and their families.
- Strong communication skills, both written and verbal, to effectively discuss progress and treatments with patients, families, and healthcare professionals.
- Ability to work independently and manage time effectively, ensuring all patients receive timely care.
- Physical ability to perform therapeutic interventions, including the capacity to maneuver equipment and assist patients physically.
- Current CPR certification.
- Valid driver’s license and reliable personal vehicle available for visiting patient homes across the service area.
