As the quarterback of our company, the Home Health Intake Specialist plays a critical role in our team. By managing the intake and scheduling of home health patients, you will ensure efficient service delivery and uphold our commitment to exceptional care. This position involves direct communication with referral sources, patients, and their families, ensuring their needs are met promptly and professionally.

Duties and Responsibilities

  • Daily management of intake processes at our office location.
  • Scheduling home health service appointments efficiently.
  • Facilitating seamless communication between referral sources, patients, and family members.
  • Maintaining comprehensive documentation and patient records in the WellSky Kinnser system.
  • Implementing strategies to optimize intake procedures and patient satisfaction.
  • Ensuring compliance with all federal, state, and local regulations pertaining to home health services.
  • Collaborating with healthcare providers to gather necessary patient information.
  • Providing exceptional customer service, addressing inquiries and concerns with the utmost professionalism.
  • Working closely with other team members to coordinate services and share relevant patient details.
  • Participating in training and workshops to stay updated on industry standards and advances in home health care practices.

Requirements

  • Proven experience in Home Health intake processes.
  • Advanced skill and familiarity with the WellSky Kinnser software system.
  • Exceptional customer service skills and the ability to handle sensitive situations with compassion.
  • Strong teamwork capabilities; ability to work harmoniously with a diverse range of healthcare professionals.
  • Excellent organizational skills, with a keen eye for detail.
  • Outstanding communication skills, both verbal and written.
  • A minimum of a high school diploma; a degree in healthcare administration or related field is preferred.
  • Ability to adapt to fast-paced environments and manage high-stress situations effectively.
  • Commitment to ethical practices and maintaining confidentiality in all patient interactions.
  • Availability to work on-site daily and flexibility to meet the scheduling needs of the organization.
