Duties and Responsibilities

  • Oversee patient care services and ensure compliance with established policies and regulations.
  • Assist in the development and implementation of strategies aimed at improving the quality of care and patient outcomes.
  • Collaborate with healthcare professionals and departments to coordinate patient care and operational activities.
  • Monitor the performance of nursing and support staff and provide guidance and professional development opportunities to foster a high-performing team environment.
  • Participate in budget formulation and ensure resources are utilized effectively to meet patient needs and organizational goals.
  • Implement innovative processes and technologies to enhance patient care and efficiency.
  • Ensure adherence to patient safety and confidentiality standards.
  • Act as an advocate for patients and families, ensuring that their concerns and needs are addressed comprehensively and respectfully.
  • Conduct regular reviews and audits of patient care services to identify areas for improvement.
  • Prepare reports and present data on patient care activities and outcomes to senior management.
  • Foster a collaborative and inclusive culture that supports the well-being and professional growth of all staff.
  • Stay abreast of developments in healthcare regulations, trends, and best practices to continually enhance the service quality.

Requirements

  • Current Registered Nurse (RN) license required.
  • Previous management experience in a healthcare setting, preferably in home health or related services.
  • Proven experience as a Care Manager or relevant administrative healthcare role.
  • Thorough knowledge of healthcare regulations and standards, including OASIS.
  • Demonstrated ability to lead and manage multidisciplinary teams efficiently.
  • Strong organizational and communication skills, capable of decision-making and problem-solving.
  • Experience with the operational and clinical aspects of patient care management.
  • Evidence of continuous professional development in healthcare leadership and management.
  • Commitment to providing high-quality, compassionate care to patients.
  • Ability to handle high-stress situations and make decisions under pressure.
  • Proficient with healthcare IT systems and software tools relevant to patient care management.
