As we expand our services to meet the growing needs of our community, we are looking for a dedicated and experienced Home Health Aide to join our full-time staff. This position is not remote, requiring hands-on patient care in their home environments across our service area.
The ideal candidate will possess a strong background in providing patient-centered care in home settings. With a focus on enhancing the quality of life and maintaining the dignity of our clients, our Home Health Aides play a critical role in our team. Duties and Responsibilities
- Provide direct patient care including bathing, dressing, mobility, and feeding as part of the daily routine.
- Monitor and record the patient's condition on a daily basis and report any changes to the supervising nurse.
- Assist with medication management by reminding patients to take their medications and helping with basic medication logistics.
- Coordinate with therapists and other medical professionals to ensure a cohesive approach to patient care.
- Support patients with basic physical therapy exercises prescribed by healthcare providers.
- Prepare and serve meals according to specific dietary guidelines and preferences of the patient.
- Provide companionship and emotional support to patients, fostering a positive and comfortable home environment.
- Attend all training sessions and team meetings to stay updated on new policies, procedures, and techniques in patient care.
- Maintain the patient's environment by performing tasks such as cleaning, laundry and other household chores.
- Assist patients with outdoor and recreational activities to enhance their physical and mental well-being.
- Respect the privacy and confidentiality of all patient information, adhering strictly to HIPAA regulations and company policies.
Requirements
- Must possess a valid Home Health Aide certificate or nursing assistant certification.
- Demonstrated ability to communicate effectively in both written and verbal forms.
- Proven track record of reliability and dependability in previous caregiving roles.
- Ability to perform all physical requirements of the position, including lifting up to 50 pounds and providing mobility support to patients.
- Strong observational skills with an attention to detail for reporting and monitoring patient health.
- Experience with electronic healthcare records and documentation software.
- Knowledge of basic medical terminology and healthcare procedures.
- Empathy, patience, and a positive, caring attitude towards the elderly, chronically ill, and disabled.
- Must pass a background check and have excellent personal and professional references.
- Current CPR certification or willingness to obtain it upon hiring.
- Valid driver’s license and reliable transportation, as the job requires travel to patient homes within the service area.
