Duties and Responsibilities

  • Conduct comprehensive patient assessments to diagnose behavioral and psychiatric conditions.
  • Initiate and maintain effective treatment plans together with healthcare team members.
  • Provide medication management and education for patients, ensuring adherence to therapeutic standards.
  • Regular home visits to monitor patient progress, adjusting care plans as necessary.
  • Documentation of patient care in compliance with legal, ethical, and institutional standards.
  • Collaborate with family members and other caregivers to educate and inform on the patient's condition and care needs.
  • Respond to crisis situations and stabilize patients using therapeutic communication and intervention techniques.
  • Maintain patient confidentiality and privacy in accordance with HIPAA regulations.
  • Participate in continuous professional development to enhance skills and knowledge in behavioral health care.
  • Advocate for patients to ensure access to necessary health care resources.

Requirements

  • Valid RN license in the state of practice.
  • Minimum of 2 years’ experience in behavioral health or mental health settings.
  • Recent experience in settings like acute psychiatric units, outpatient services, or psychiatric home care is preferred
  • Experience with Dementia/Alzheimer Patients
  • Experience with home health care services is highly desirable.
  • Advanced knowledge of psychiatric nursing principles and protocols.
  • Ability to work independently and manage a caseload effectively.
  • Strong interpersonal and communication skills, capable of effectively managing sensitive situations.
  • Familiarity with electronic health record (EHR) systems and proficient in clinical documentation.
  • Current CPR certification.
  • Valid driver’s license and dependable transportation for home visits.
  • A strong commitment to patient safety, privacy, and dignity.
  • Flexible schedule, including availability for evening or weekend visits if required.
