Job Description

h/care, a leader in the provision of compassionate, high-quality hospice care, is excited to announce an opening for a dedicated and experienced Hospice Aide specializing in Hospice care. This is a pivotal role within our organization, aimed at enhancing the daily lives of our patients who are navigating the complexities of end-of-life care. Integral to our team, the successful candidate will join a group of caring professionals in a full-time capacity, helping to maintain our standard of excellence in the support and comfort we provide.

Our Hospice Aides are essential in embodying the warmth and professionalism our clients have come to expect. Please note, this is a full-time position based in our hospice care facilities and the surrounding community; it is not a remote job. Applicants should be prepared for in-person interactions with a focus on contributing effectively through empathy, detailed attention, and uncompromised support.

Duties and Responsibilities

  • Provide direct patient care as directed by the nursing staff, adhering to the established plan of care with attention to special needs and personal preferences of the client.
  • Assist with the basic activities of daily living, including but not limited to, dressing, bathing, grooming, eating, and maintaining overall patient hygiene.
  • Monitor and record the patient’s condition, reporting any changes to the registered nurse or supervising healthcare professional in a timely manner.
  • Support a safe and clean environment by adhering to infection-control policies and protocols, medication administration, storage procedures, and controlled substance regulations.
  • Ensure equipment operation by following the manufacturer's instructions, troubleshooting malfunctions, calling for repairs, and evaluating new equipment and techniques.
  • Maintain patient confidence and protect operations by keeping personal information confidential in compliance with the Privacy Act, HIPAA, and organization confidentiality policies.
  • Provide compassionate presence, emotional support, and personal care to patients, making them as comfortable as possible in their last days, focusing on palliative care needs and family dynamics.
  • Actively participate in multidisciplinary team meetings, offering insights and observations regarding the patient's needs and responses to care.
  • Enhance hospice reputation by accepting ownership for accomplishing new and different requests, exploring opportunities to add value to job accomplishments.
  • Provide bereavement support to families and caregivers facing grief and loss.

Requirements

  • Certification as a Home Health Aide or Nurse’s Assistant; certification specific to hospice or palliative care preferred.
  • Minimum of two years of experience in hospice care, palliative care, or a similar setting dealing with end-of-life care.
  • Demonstrated ability to communicate effectively, both orally and in writing, with patients, families, and other healthcare providers.
  • Deep understanding of the principles of personal care, patient hygiene, and safety.
  • Knowledge of HIPAA regulations and an ability to maintain strict confidentiality of personal health information.
  • Strong observational skills with an ability to recognize changes in patient conditions.
  • Physical ability to perform the duties, including lifting patients, bending, standing for long periods, and providing physical support to patients.
  • Empathy and interpersonal skills with an ability to care for patients from diverse backgrounds and various mental and physical states.
  • Valid driver’s license and reliable transportation; willing to travel within the community to visit patients in their homes.
  • Current CPR certification or ability to acquire it prior to job commencement.
