Job Description

We at h/care are excited to announce an immediate opening for a dedicated and passionate Licensed Vocational Nurse (LVN) specializing in Home Health. This position is ideal for a professional who is ready to take on the challenges and rewards of home healthcare, providing direct nursing care to various patients in their own homes. Our Home Health LVNs play a crucial role in our team, working full-time to ensure that all patients receive the highest standard of care tailored to their specific needs. This is a full-time position and is not eligible for remote work.

As an LVN in Home Health at h/care, you will be joining a dynamic team of healthcare professionals who are committed to making a significant difference in the lives of patients. This role requires a compassionate, detail-oriented, and proactive nursing professional who is ready to handle the various demands of home healthcare nursing. Our LVNs ensure that the health and well-being of each patient are met through comprehensive and personalized care planning and delivery.

Duties and Responsibilities

  • Conduct visits in the patients home to assess the patients' health status and contribute to patient care planning and documentation.
  • Administer medications, perform routine wound care and document the patients' response to treatments.
  • Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care.
  • Monitor patient health status and provide updates to the care team and family members as appropriate.
  • Educate patients and their families on disease processes, medical surgical care and other health wellness topics.
  • Implement infection control measures according to organizational policies and regulatory compliance.
  • Assist with activities of daily living (ADLs), ensuring that patients maintain their dignity and quality of life.
  • Manage emergency situations with competence, calling for assistance when needed.
  • Provide emotional support to patients and their families, helping them cope with the challenges of illness and recovery.
  • Maintain patient confidentiality and uphold the privacy laws (HIPAA).
  • Coordinate with healthcare providers, third-party vendors, and insurers to advocate for the patient’s health care needs.
  • Ensure accurate maintenance of patient records and compliance with reporting requirements.

Requirements

  • Current license as a Licensed Vocational Nurse in the state of practice.
  • Minimum of two years' experience in nursing, preferably in home health or related field.
  • Demonstrated ability to assess and respond to the needs of patients and families in diverse communities.
  • Knowledge of clinical protocols, procedures, and medical terminology relevant to nursing practice.
  • High level of proficiency in managing EMR (Electronic Medical Records) and other technology-based healthcare tools.
  • Excellent communication skills, both verbal and written, for effective interaction with all levels of healthcare staff and patients.
  • Ability to work under pressure and react effectively in emergency situations.
  • Valid driver's license and reliable transportation necessary for travel to patient homes within the service area.
  • Physical endurance to perform tasks over long periods including lifting up to 25 lbs, bending, standing, and walking.
  • Commitment to ongoing professional development and continuing education.
  • Proof of current CPR (Cardiopulmonary Resuscitation) and first aid certification.
  • Demonstrate ethical practice and knowledge of state and federal regulations regarding home health care.
