Job Description

We at h/care are currently seeking a dedicated and compassionate individual to fill the role of Experienced Home Health Aide. This is a part-time to full-time position that requires the aide to be present on-site, as this is not a remote job. Our ideal candidate will have substantial experience in hospice care and possess a deep understanding of the requirements and sensitivities involved in caring for ill patients. As a home health aide, you will play a critical role in enhancing the quality of care provided to our patients and assisting with adls/iadls

Duties and Responsibilities

  • Provide direct care to patients in a home health setting, ensuring their ability to perform adls/iadls
  • Assist patients with daily activities such as bathing, dressing, and grooming.
  • Manage routine healthcare tasks like checking vital signs including blood pressure, pulse, and respiration.
  • Administer medication as prescribed by the attending healthcare provider and monitor side effects.
  • Support family members in understanding care procedures and interventions.
  • Maintain patient care records, document actions by completing forms, reports, logs, and records.
  • Collaborate with a wider team of healthcare professionals to ensure continuity of care.
  • Offer end-of-life care and support to patients and provide comfort measures appropriate to individual needs.
  • Utilize knowledge of disease progression to anticipate changes in patient's condition and needs.
  • Respect all confidentiality and privacy requirements of the patients and their families.
  • Attend and participate in scheduled training and educational workshops to maintain updated skills and knowledge.
  • Ensure a safe, clean, and organized environment for both the patient and family.

Requirements

  • Certification as a Home Health Aide or Nursing Assistant.
  • Previous experience in home health or hospice environment preferred.
  • Demonstrated ability to communicate effectively in both oral and written forms.
  • Proficient in following complex instructions.
  • Strong observational skills and ability to assess clinical situations promptly.
  • Physical ability to perform all duties of the job including lifting, standing, bending, and transferring patients.
  • Ability to work independently and within a team with minimal supervision.
  • Knowledge of hygiene and infection control standards.
  • Respectful and empathetic approach to patients, families, and staff.
  • Understanding of palliative care philosophies and practices.
  • Must pass a criminal background check and undergo health screenings.
  • Reliable transportation and willingness to travel to patients' homes within the local area.
