h/care, a leader in home health services, is excited to announce a new opportunity for a dedicated Full-Time Experienced Home Health Start of Care RN to join our team. This is an in-person, non-remote position requiring a compassionate and professional nurse to manage initial in-home health assessments and care planning for our clients. This role is crucial in setting the tone for the quality and effectiveness of ongoing home health services, contributing directly to patient outcomes and satisfaction.

As a Start of Care RN, you will play a pivotal role in our team by initiating home health services following a client’s discharge from a hospital or facility. Your expertise in assessing patient needs, developing care plans, and documenting accurately on OASIS will drive the success of patient home health set-up and ensure compliance with all regulatory and internal standards.

Duties and Responsibilities

  • Conduct comprehensive initial in-home assessments of patient’s health and environment after discharge from hospitals or other facilities.
  • Develop individualized patient care plans in collaboration with physicians, patients, and other healthcare professionals.
  • Document patient care assessments and interventions using OASIS to ensure accurate data collection for outcome-based care management.
  • Initiate the coordination of services with multifaceted healthcare teams to provide continuous quality care.
  • Educate patients and their families on disease processes, treatment, home health care procedures and other essential concerns related to their care plan.
  • Monitor patient progress and make necessary adjustments to the care plan based on evaluative findings and consultations with healthcare team members.
  • Ensure compliance with all federal, state, and local regulations as well as company policies regarding patient care and privacy.
  • Participate in professional development activities to improve knowledge and performance to sustain high standards of care.
  • Maintain communication with health care team members regarding changes in patient’s condition, care plan adjustments, and other relevant concerns.
  • Provide emotional support and compassionate care to patients and family members during the critical initial phase of home care setup.

Requirements

  • Valid RN license in the state of employment.
  • Minimum 2 years of nursing experience, preferably in home health or related fields.
  • Demonstrated ability to perform comprehensive patient assessments and document findings in OASIS.
  • Strong knowledge of disease management and home health care necessary.
  • Exceptional organizational skills and able to handle multiple priorities effectively.
  • High proficiency in using healthcare software and computer systems.
  • Excellent communication and interpersonal skills to interact with a diverse group of patients, healthcare providers, and family members.
  • Ability to travel to patients’ homes within designated service areas.
  • Commitment to ethical standards and confidentiality of health information.
  • Current CPR certification.
