Job Description

VNA of OHIO is excited to announce an opening for a dedicated and skilled Experienced Occupational Therapist to join our Home Health team on a Full-Time basis. This position is integral to our commitment to provide high-quality rehabilitative care to patients in their own homes. As we focus on helping patients maintain or regain their independence, this role involves working closely within the patients' personal environments to deliver the best possible outcomes. Please note that this is not a remote job. The role requires travel within Greater Cleveland to offer personalized care directly where it's most effective.

As an Occupational Therapist with VNA of OHIO, you will have the unique opportunity to make a tangible difference in the lives of your patients by assessing their daily routines and customizing intervention plans that promote optimal function and wellness. Duties and Responsibilities

  • Conduct thorough patient assessments to determine their functional status and their specific rehabilitation needs.
  • Design and implement personalized intervention strategies that facilitate patient engagement and foster independence.
  • Modify the patient's home environment to enhance their ability to perform daily tasks safely and efficiently.
  • Provide training and education to family members and caregivers to ensure a supportive, therapeutic environment.
  • Regularly communicate with a multidisciplinary team, including nurses, physical therapists, and social workers, to coordinate and optimize patient care.
  • Document patient care and progress thoroughly in the electronic medical record (EMR) system.
  • Assess and adapt interventions as patient needs evolve, demonstrating flexibility and innovative problem-solving.
  • Adhere to all state and federal regulations regarding occupational therapy and patient privacy.
  • Stay current with best practices in occupational therapy and participate in ongoing education and training.
  • Maintain a positive and empathetic demeanor, with a strong commitment to enhancing patient quality of life.

Requirements

  • Current Ohio Occupational Therapist license in good standing.
  • CPR/BLS certification must be active before the start date.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • Minimum of 6 months of OT experience, preferably in a home health setting; however, candidates from hospital, SNF, or outpatient settings are also welcome.
  • Strong clinical reasoning and problem-solving skills with the ability to adapt plans in a home setting.
  • Ability to work independently while remaining a collaborative part of an interdisciplinary care team.
  • Comfort with technology and proficiency in electronic medical record documentation.
  • Willingness to serve a diverse patient population across Cleveland and surrounding communities.
  • Commitment to patient-centered care and aligning treatment goals with the patient’s real-life needs and environment.
