The Role

As a Mental Health RN Case Manager in our home health program, you’ll be the heart of our mission, delivering expert, compassionate care to patients with mental health needs in their homes. You’ll assess, plan, and coordinate care, empowering patients to achieve stability and independence in their own environments.

Key Responsibilities:

  • Perform in-home mental health assessments to create individualized care plans tailored to each patient’s needs.

  • Provide skilled nursing care, including medication administration, symptom monitoring, and crisis intervention.

  • Coordinate with physicians, therapists, and community resources to ensure seamless, patient-centered care.

  • Educate patients and families on mental health conditions, treatment plans, and coping strategies to promote wellness.

  • Maintain accurate, timely documentation in compliance with home health regulations and standards.

  • Advocate for patients, connecting them with community services and support systems to enhance their quality of life.

Who We’re Looking For

You’re a skilled RN who’s passionate about mental health and tired of environments that limit your ability to connect with patients. You want a role where you can provide hands-on, meaningful care in the home, with the autonomy to make a difference and the support to thrive.

Qualifications:

  • Active Ohio RN License in good standing.

  • At least 1 year of mental health nursing experience (home health or case management experience preferred).

  • Strong understanding of psychiatric conditions, treatments, and community resources.

  • Excellent communication and interpersonal skills to build trust with patients and families in diverse home settings.

  • Independent problem-solving skills with the ability to make sound clinical decisions in the field.

  • Valid driver’s license and reliable transportation (travel within Western Cuyahoga and Lorain Counties required).

  • BLS certification required; psychiatric nursing or home health certification a plus.
