VNA of Ohio is hiring a mission-driven LISW to support both our Home Health and Community Mental Health programs. You'll provide in-home psychosocial assessments, short-term counseling, and care coordination for patients recovering from illness or managing chronic mental health conditions.

What You'll Do:
In-home assessments and brief therapy for home health patients
Ongoing mental health therapy for adult clients (Medicaid)
Collaborate with nurses, therapists, and case managers
Support transitions, resources, and whole-person care
What We’re Looking For:
Active Ohio LISW (LISW-S a plus)
2+ years clinical experience (home-based or CMH preferred)
Strong clinical judgment, documentation, and empathy
