About the Role: We’re hiring a Director of Community Partnerships to lead referral and growth strategy across all three service lines. This isn’t just a sales role - it’s a leadership position with direct input on market strategy, team performance, and how we grow our presence across Phoenix. You’ll work closely with executive leadership and experienced clinical partners who stand ready to admit patients quickly and support your efforts in the field.
You’ll also lead a focused, accountable sales team while building deep relationships with hospital systems, physicians, senior communities, and others who need a trusted partner to refer to.
What You’ll Do
Own referral growth across Home Health, Palliative, and Hospice lines of service.
Build high-trust relationships with hospitals, physicians, senior living communities, and care navigators.
Leverage your market knowledge to identify growth opportunities and remove referral friction.
Promote our in-home NP provider model and explain how our full continuum supports patients and reduces avoidable hospitalizations.
Lead, coach, and develop a high-performing sales team.
Collaborate closely with clinical leadership to ensure patients are admitted quickly and cared for consistently.
Use CRM and data tools to track pipeline, performance, and accountability.
Represent h/care confidently at meetings, events, and in the community.
What We’re Looking For
Track record of success in hospice, home health, or palliative care sales, with strong knowledge of referral development.
Deep relationships in the Phoenix provider network—especially with hospitals and senior communities.
Experience leading or mentoring a sales team.
Strategic thinker with strong follow-through. You know how to plan your work and work your plan.
Strong communicator who earns trust quickly and follows through reliably.
Passion for improving patient care at home and working with clinical partners to make that happen.
Comfortable using CRM systems and metrics to drive performance.
