As an Experienced Hospice LPN at h/care, you will be an integral part of our patient care team, focused on delivering quality nursing practices alongside emotional support tailored to each patient's needs. This role requires a combination of medical expertise, compassion, and excellent interpersonal skills to ensure a comforting environment for both patients and their families.

Duties and Responsibilities

Provide direct nursing care in accordance with the patient’s care plan consistent with the scope of the state's Nurse Practice Act.
Monitor patient’s condition and notify appropriate personnel of any changes.
Administer prescribed medications, change dressings, clean wounds, and monitor vital signs.
Collaborate with the nursing team to optimize patient care and comfort using a holistic approach.
Maintain accurate patient medical records and document nursing interventions.
Educate patients and their families on disease processes, medical-surgical procedures, and other aspects of therapeutic regimens.
Advocate for the patient's comfort and satisfaction, adjust care strategies accordingly.
Engage in decision making related to the appropriateness of healthcare interventions.
Facilitate timely communication between patients, family members, and the multidisciplinary hospice care team to ensure continuity of care.
Participate in hospice quality assurance and performance improvement initiatives.
Undertake ongoing formal and informal education to maintain and enhance nursing skills and knowledge.

Requirements

Must hold an active LPN license in the state of employment.
Minimum of two years’ experience in a hospice or related health care facility.
Dependable with a commitment to maintain scheduled hours required.

Exceptional bedside manners, reflecting empathy and commitment to patient care.
