This position is located in Cleveland, OH, and offers the opportunity to join a dynamic team committed to high standards of healthcare delivery. The ideal candidate will bring a strong background in mental health and in-home healthcare experience. Please note, this is not a remote job; the successful applicant will be required to work on site as part of a team dedicated to managing behavioral health patients effectively.

As an Experienced RN Mental Health - Case Manager, you will play a critical role in our integrated healthcare team, delivering personalized and effective care plans for each patient. This role demands a combination of clinical expertise, compassionate patient interaction, and detailed case management. Duties and Responsibilities

  • Assess patient health problems and needs, focusing on mental and behavioral aspects.
  • Develop and implement nursing care plans for mental health patients, adjusting them as necessary during treatment.
  • Work collaboratively with psychiatrists, therapists, and other healthcare professionals to fine-tune therapy and medication plans.
  • Provide emotional support to patients and their families, educating them about conditions, and the steps towards recovery.
  • Monitor, record, and report symptoms and changes in patient behavior and condition.
  • Coordinate and monitor multiple cases simultaneously, ensuring all are handled efficiently without compromising care quality.
  • Facilitate group therapy sessions and provide individual counseling as required.
  • Manage patient documentation carefully and comply with all legal and professional standards.
  • Participate in continuous training and development to keep abreast of innovations in mental health care.
  • Implement strategies to aid in the de-escalation of potentially volatile situations with sensitivity.
  • Advocate for patient needs and preferences with health care providers to ensure optimal patient-centered outcomes.

Requirements

  • Valid RN license in the state of Ohio.
  • Proven experience as a home health mental health nurse case manager.
  • Extensive knowledge and clinical experience in behavioral and mental health disorders.
  • Strong interpersonal skills with the ability to manage highly stressed situations.
  • Excellent communication skills, both written and verbal.
  • Ability to work effectively within a team and also independently in a challenging environment.
  • Demonstrated proficiency in clinical assessments and the ability to develop detailed care plans.
  • Committed to continuous education and training in mental health fields.
  • Compassionate and empathetic demeanor with a patient-centric approach.
  • Availability to handle rotational shifts as required by the facility.
