HOME HEALTH PHYSICAL THERAPIST JOB DESCRIPTION
VNA of Ohio, Cleveland, Ohio (reports to Clinical Supervisor, Home Health)
Role Overview:– Restore patient mobility and reduce pain at home– Build trust through empathetic communication
Key Responsibilities:
– Conduct detailed assessments
– Deliver 30 productivity points per week
– Teach home exercise and fall-prevention strategies
– Mentor therapy assistants
Typical Day:
8 AM – Chart review and equipment check
9 AM–12 PM – Patient visits (3–4 homes)
12 PM–1 PM – Lunch and quick documentation
1 PM–3 PM – Afternoon visits (2–3 homes)
3 PM–3:15 PM – Friday team huddle: share wins and troubleshoot
3:15 PM–5 PM – Finalize notes and prep for tomorrow
Success Metrics:
– 30 points per week on our productivity scale
– Patient satisfaction scores ≥ 4.5 out of 5
– 80% of patients meet functional goals within four weeks
– Zero late or incomplete notes
Qualifications:
– DPT or bachelor’s in physical therapy plus active Ohio PT license
– 2+ years home-care or similar patient-centered experience
– Strong interpersonal and teaching skills
– Comfortable driving 30–40 miles/day (IRS-rate reimbursed)
Technology & Tools:
– Proficient in HCHB EHR
– Company-issued tablet and clinical kit provided
