Job Description

h/care, a leading provider in hospice care, is excited to announce a vacancy for the position of Experienced Registered Nurse Case Manager (RNCM) specializing in Hospice care. We are seeking a dedicated and compassionate individual to join our team in a full-time capacity. This important role is based out of our facility and requires onsite presence, as it is not a remote job. The successful candidate will be instrumental in providing end-of-life care, ensuring patients receive the highest quality of comfort and support during their final stages.

As an RNCM at h/care, you will handle a small caseload of patients, overseeing and coordinating their entire care journey. This role is pivotal in making meaningful differences in the lives of patients and their families, upholding our commitment to respectful and dignified care. Duties and Responsibilities

  • Develop comprehensive and individualized care plans for each patient.
  • Conduct regular visits to patients' homes or care facilities to provide clinical assessments and updates to care plans as needed.
  • Manage symptoms efficiently through advanced clinical skills, ensuring patient comfort at all times.
  • Educate and support the families of patients, providing them with the information needed to understand their loved one’s condition and care.
  • Collaborate with a multidisciplinary team including aides, social workers, chaplains, and physicians to effectively coordinate end-of-life care.
  • Ensure compliance with all relevant healthcare laws, regulations, and hospice guidelines.
  • Maintain accurate and detailed patient records through our EMR system, aiding in the seamless continuity of care.
  • Participate in on-call rotations to provide critical support and care during emergencies outside of regular hours.
  • Continually update professional knowledge and participate in educational opportunities relevant to hospice care.

Requirements

  • Active RN license in the state of Arizona.
  • Minimum of one year of nursing experience, with hospice or home health settings strongly preferred.
  • Strong clinical assessment skills and comfort with independent decision-making in the field.
  • Working knowledge of hospice eligibility guidelines and effective symptom management strategies.
  • Reliable transportation, a valid driver’s license, and up-to-date auto insurance.
  • Experience with EMR systems, with preference given to candidates familiar with WellSky/Kinnser.
  • Excellent communication skills and a high level of emotional intelligence to interact effectively with patients and families.
  • Ability to participate in the on-call rotation, typically required 1–2 times per month.
