h/care, a dedicated leader in end-of-life care, is excited to announce a unique job opportunity for a passionate individual to join our team as a Community Hospice Advocate - Consultant 1099. This position offers the flexibility of a contract/temporary role accommodating a work-from-home (WFH) arrangement, suitable for those who excel in autonomous and thoughtful work environments. Our aim is to extend empathetic, comprehensive hospice care to individuals and families navigating the challenges of life-limiting illnesses. As a consultant in this pivotal role, you will represent h/care and uphold our values of dignity, compassion, and ethical care delivery.

Ideal candidates for this position are those currently working in Assisted Living Facilities or Memory Care facilities with a clear understanding of the hospice philosophy. As a Community Hospice Advocate, you will play a crucial role in identifying potential hospice candidates and coordinating the initial meetings to discuss the benefits of timely admissions to hospice care. This role requires a deep passion for end-of-life care and a commitment to advocating for the best possible outcomes for our clients and their families.

Duties and Responsibilities

  • Identify and engage potential hospice care candidates within Assisted Living and Memory Care facilities.
  • Coordinate and conduct introductory meetings between hospice care teams and potential clients and their families.
  • Advocate for the benefits of timely hospice admission, demystifying the process and addressing any concerns or misconceptions.
  • Maintain thorough knowledge of hospice care regulations, philosophy, and services to provide accurate information and support.
  • Facilitate seamless communication between healthcare providers, patients, and hospice care teams to ensure coordinated care delivery.
  • Monitor and report on the efficacy of hospice advocacy efforts and suggest improvements based on feedback and outcomes.
  • Participate in community outreach programs and educational sessions to raise awareness about the benefits of hospice care.
  • Ensure compliance with all federal, state, and local regulations and ethical standards pertaining to hospice care advocacy.

Requirements

  • Proven track record of effective advocacy and care coordination in Assisted Living or Memory Care facilities.
  • In-depth understanding of hospice philosophy and the willingness to promote its benefits to communities and individuals.
  • Excellent customer service skills, with the ability to communicate compassionately and effectively with diverse groups of people.
  • Strong organizational skills and the capability to manage multiple cases efficiently in a work-from-home setting.
  • Education in healthcare, social work, or a related field, with specific training or certification in hospice care preferred.
  • Demonstrated ability to work independently as a contracted consultant (1099) with a consistent track record of meeting or exceeding professional goals.
  • Availability to travel locally as needed to meet with clients and participate in community events.
