This Full-Time position is crucial for the expansion and reinforcement of our community relations and outreach initiatives. The successful candidate will helm the development of strategic partnerships with local hospitals, physicians, and community groups, directly impacting our growth and presence in the market. Located in Phoenix, this role is Not a Remote Job. The Director of Community Partnerships is not only a role that requires outstanding personal skills and profound market insights but also demands a passionate commitment to the healthcare industry. The primary goal is to drive patient growth by transforming potential community liabilities into strong, reliable referral partners. This position involves setting strategic market plans, mentoring a focused sales team, and meticulously tracking critical metrics to ensure our services effectively reach and impact families across Phoenix.

Duties and Responsibilities

  • Develop and execute a comprehensive market strategy focused on increasing patient referrals from hospitals, physicians, and community groups.
  • Leverage personal networks and deep market insights for strategic advantage and partnership development.
  • Mentor and lead a small, dedicated sales team to meet and exceed assigned admission and revenue targets.
  • Establish fruitful and sustainable partnerships with community leaders and healthcare providers to increase service footprint.
  • Monitor performance indicators and adjust outreach strategies as needed to achieve targets.
  • Represent the organization at community and industry events to boost brand awareness and engagement.
  • Ensure all commitments made in the field translate into exceptional care for patients at home, strengthening our institution's reputation.
  • Regularly report on market insights, partnership development progress, and achievements towards overall growth objectives.

Requirements

  • Proven experience in community partnership roles or similar within the healthcare sector.
  • Strategic thinker who can spot market trends, set clear growth plans, and adjust tactics quickly when data shifts.
  • Proven accountability for hitting tough admission and revenue targets, with transparent reporting and follow-through.
  • Strong leadership skills, able to coach a small sales team, inspire cross-functional partners, and model the behavior you expect from others.
  • Excellent communication and interpersonal skills, capable of building and sustaining high-level relationships.
  • Mastery in negotiating and securing partnership agreements that align with organizational objectives.
  • Robust analytical skills to interpret market data effectively and make data-driven decisions.
  • Ability to manage multiple priorities in a dynamic, fast-paced environment.
