This hands-on role is ideal for a compassionate, creative, and resourceful medical social worker looking to make a significant impact within the Oregon coast community. As part of our commitment to expand and enhance hospice services in this beautiful region, we are seeking a professional who is deeply rooted in the community and motivated to be at the forefront of a new hospice initiative. Please note, this is an on-site position and is not eligible for remote work.

Duties and Responsibilities

  • Conduct comprehensive assessments of patient and family psychosocial needs to integrate into the hospice plan of care.
  • Facilitate advance care planning and discussions on end-of-life options with patients and family members.
  • Provide emotional support and counseling to patients and their families, facing the emotional, social, and financial challenges of end-of-life scenarios.
  • Act as a liaison between patients, families, and healthcare facilities, ensuring seamless communication and service delivery.
  • Develop and oversee a bereavement program, including support groups and memorial services, tailored to the needs of families post-hospice care.
  • Coordinate and manage volunteers, including recruitment, training, and assignment of duties to support hospice operations and patient needs.
  • Maintain comprehensive documentation of patient interactions, assessments, and care plans in compliance with regulatory standards.
  • Advocate for patient rights and self-determination within the confines of hospice care regulations and personal capabilities.
  • Stay informed of the latest developments in hospice care practices and social work to continually enhance service delivery.
  • Participate in interdisciplinary team meetings to review and update patient plans of care and adapt to changing patient needs.
  • Assist families in understanding and navigating healthcare options and community resources, leading to informed and empowered care decisions.
  • Support the training and development of new staff or interns by providing mentorship and guidance in hospice social work practices.
  • Design and implement community outreach programs to enhance public understanding and support of hospice care services.

Requirements

  • Master’s degree in Social Work (MSW) required, preferably with a focus in healthcare or palliative care.
  • Licensed Clinical Social Worker (LCSW) in the state of Oregon or eligible for licensure.
  • Minimum of three years’ experience in hospice care, palliative care, or a related healthcare setting.
  • Strong roots in the Oregon coast community with a keen interest in community health issues and dedicated to service improvement.
  • Proven ability to handle complex emotional situations related to end-of-life care and to provide compassionate support and guidance.
  • Excellent communication and interpersonal skills, capable of managing diverse groups of patients, families, and professionals.
  • Demonstrated leadership in managing volunteer groups and experience in bereavement counseling.
  • Must be resourceful, creative in problem solving, and capable of utilizing available resources efficiently to meet patient and organizational needs.
  • Commitment to the values of dignity, integrity, and confidentiality in all professional conduct.
