Job Description

We are a dedicated provider of hospice care, is excited to offer a career opportunity for a compassionate and skilled Experienced Registered Nurse Case Manager to join our team. This full-time position plays a crucial role in supporting our mission to deliver exceptional end-of-life care tailored to meet the needs of each patient and their families. This is not a remote job and will require the candidate to be based and actively participate in the local community of the Oregon Coast region.

Duties and Responsibilities

  • Conduct comprehensive assessments of patient and family needs to create and implement detailed, personalized care plans.
  • Coordinate and supervise the care delivered by other healthcare professionals, ensuring adherence to established standards and hospice philosophy.
  • Provide direct nursing care when needed, especially during critical periods or for symptom management.
  • Maintain regular communication with patients, families, and team members to ensure all parties are well-informed and that care plans are adjusted as necessary.
  • Advocate for patient needs and wishes with healthcare providers, support services, and external resources to ensure a dignified hospice experience.
  • Educate patients, families, and community members about hospice care principles, palliative care options, and end-of-life planning.
  • Participate in on-call rotations to address urgent or emergent patient needs outside of regular working hours.
  • Document all patient interactions and interventions accurately and punctually for legal, medical, and billing purposes.
  • Lead and participate in team meetings and case reviews to ensure a cohesive and unified approach to patient care.
  • Support the continuous improvement of organization standards through professional development and adherence to best practices in hospice care.

Requirements

  • Current RN license in good standing with the state board of nursing.
  • Minimum of two years of experience in hospice or palliative care nursing.
  • Demonstrated ability to develop and manage comprehensive care plans effectively.
  • Excellent communication and interpersonal skills, capable of handling sensitive situations with empathy and professionalism.
  • Strong organizational skills and the ability to multitask effectively in a fast-paced environment.
  • A deep-rooted connection to the Oregon Coast community, with a commitment to serve and enhance local healthcare services.
  • Profound knowledge of the hospice care industry and end-of-life care standards.
  • Ability to work collaboratively in a team and independently with minimal supervision.
  • Eagerness to undertake multiple responsibilities and roles within the agency, aiding in its growth and success.
  • Passion for hospice care and a driven attitude to contribute significantly to the establishment and success of a new care provider.
