  • Patient Assessment and Goal Setting: Conduct meaningful assessments to understand patients' unique needs and goals, crafting treatment plans that empower them to regain independence and confidence in their daily lives.
  • Holistic Interventions: Implement creative and evidence-based therapeutic techniques, such as home adaptations, functional skill training, and adaptive equipment use, to help patients thrive in their home environments.
  • Patient and Family Engagement: Build strong relationships with patients and families, offering encouragement and education that inspires collaboration and sustained progress.

  • Seamless Teamwork: Partner with nurses, physical therapists, and care teams to create cohesive, patient-focused care plans that address the whole person, not just their condition.

A Typical Day

  • Morning Preparation: Start the day by reviewing your patient schedule and treatment plans. Prepare any necessary equipment or materials to deliver tailored interventions for each patient.
  • Patient Visits and Therapy: Travel to patients’ homes to provide hands-on therapy, focusing on improving their functional abilities and independence. Address areas such as mobility, daily living skills, and home environment adaptations while building strong rapport with patients and their families.
  • Mid-Day Collaboration: Take time to consult with interdisciplinary team members, including nurses and physical therapists, to discuss patient progress and refine care plans for optimal outcomes.
  • Afternoon Follow-Ups and Documentation: Continue with patient visits, ensuring each session is personalized to meet individual goals. Afterward, document treatment progress and update care plans as needed, adhering to regulatory and organizational standards.
  • Patient and Family Education: Educate patients and their families on therapeutic techniques, use of adaptive equipment, and strategies for maintaining progress between visits. Empower them to play an active role in achieving long-term goals.
  • Community Engagement: Participate in local health-related events or initiatives to advocate for the role of occupational therapy in home health care and build awareness about its benefits.
  • Evening Reflection and Planning: Review the day’s accomplishments and plan for the following day, ensuring that each patient’s needs are prioritized and treatment plans are fully prepared.

 

 

 

Your Expertise

  • Education: Bachelor’s or Master’s degree in Occupational Therapy from an accredited program. Must hold a current state licensure as an Occupational Therapist.
  • Experience: At least one year of clinical experience in occupational therapy, preferably in a home health or community-based setting. Experience working with diverse populations and addressing a variety of physical and cognitive challenges.
  • Therapeutic Expertise: Proficient in developing and implementing individualized care plans, including functional skill training, adaptive equipment recommendations, and home modifications to enhance patient independence.
  • Regulatory Knowledge: Strong understanding of home health care regulations, including CMS’s Conditions of Participation and HIPAA, ensuring compliance in all aspects of patient care.
  • Communication Skills: Exceptional ability to communicate effectively with patients, families, and interdisciplinary care teams, fostering trust and collaboration throughout the care process.
  • Patient-Centered Focus: Deep commitment to providing compassionate, holistic care that empowers patients to achieve their goals and improve their quality of life.
  • Problem-Solving Skills: Skilled in identifying barriers to progress and developing creative solutions to help patients overcome challenges in their home environments.

Your Work Environment

  • Location: Home-based with extensive travel throughout the assigned area to provide care directly in clients’ homes. Occasional team meetings may be held in the Cleveland office.
  • Pace: A patient-centered and dynamic environment with a focus on providing individualized care, meeting productivity goals, and delivering exceptional outcomes.
