The position is based on-site and requires the nurse to work closely with patients and their families in their homes or care facilities, managing complex health conditions with compassion and expertise. As an Experienced Registered Nurse Case Manager at h/care, you will join a team of skilled healthcare professionals dedicated to a patient-centered approach, ensuring that each patient's final days are as comfortable as possible. The position demands a high level of expertise in managing both the medical and emotional needs associated with hospice care.

Duties and Responsibilities

  • Assess patient needs during regular visits, and adjust care plans as necessary in consultation with hospice physicians.
  • Administer and advise on pain management techniques, ensuring patient comfort.
  • Coordinate with an interdisciplinary team including physicians, social workers, and other healthcare professionals to provide holistic care to patients.
  • Document patient status and care provided in electronic medical records accurately and comprehensively.
  • Provide emotional and practical support to patients' families, educating them on end-of-life care strategies and expectations.
  • Manage a caseload of patients, scheduling visits and ensuring all team members are informed of any changes in patient condition.
  • Participate in professional development and in-service training to stay current with the latest advancements in hospice care practices.
  • Ensure all care practices are compliant with state and federal regulations pertaining to hospice and palliative care.
  • Maintain communication with families to provide updates and understand their needs and expectations.
  • Support the emotional needs of patients and families, providing guidance and reassurance throughout the hospice journey.

Requirements

  • Active Registered Nurse (RN) license in the state of Arizona.
  • Minimum of 2 years of hospice or palliative care experience.
  • Strong clinical skills in pain and symptom management.
  • Proficiency in electronic medical record (EMR) documentation.
  • Ability to participate in after-hours on-call rotation.
  • Reliable transportation and a valid driver's license.
  • Certification in Hospice and Palliative Nursing (CHPN) preferred.
  • Strong communication skills and emotional resilience.
  • Commitment to patient-centered, culturally competent care.
