Job Summary: We are seeking a dedicated and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team on a contract basis. This role carries primary responsibility for medical and psychiatric functions in an outpatient setting, including diagnosis, psychiatric evaluations, and medication management. The ideal candidate is committed to person-centered care and capable of working collaboratively in a community-based mental health environment.

Responsibilities:

  • Carry primary responsibility for medical and psychiatric functions in an outpatient clinical setting (diagnosis, psychiatric evaluations, and medication services)
  • Provide general psychiatric services including engagement, assessment, differential diagnosis, treatment planning, medication management, family meetings, supportive psychotherapy, patient education, risk assessment, and discharge planning
  • Ensure all psychiatric services are delivered in compliance with licensing and funding requirements as outlined in the Covered Services Guide
  • Conduct psychiatric assessments, medication monitoring, and support continuity of care using a client-directed, outcome-informed approach
  • Provide oversight and direct supervision as needed, including review and revision of procedures related to medically-related daily activities
  • Maintain accurate and compliant medical chart documentation, with emphasis on coordination of care with primary care providers (PCPs), lab result monitoring, and vitals compliance standards
  • Strong communication skills with the ability to build rapport with patients from diverse backgrounds.
  • Ability to work effectively both independently and as part of a team
