Duties and Responsibilities

- Collaborate with other healthcare professionals to coordinate treatment plans and provide comprehensive patient care.
- Maintain accurate patient records and document patient care services by entering information into patient medical records.
- Guide and educate patients and their families about home health techniques, preventative measures, and treatment plans.
- Manage rehabilitation equipment and assist in training staff and new hires in equipment and therapy techniques.
- Stay updated with advancements in the field of physical therapy and integrate evidence-based practices into treatment protocols.
- Assess treatment outcomes and modify treatment plans accordingly to achieve the best possible health improvements.
- Comply with federal, state, and local certification requirements, ensuring all practices are compliant and up to professional standards.
- Provide motivational support and guidance to patients to achieve therapy goals and improve overall well-being.
Requirements
- Master's or Doctorate degree in Physical Therapy from an accredited program.
- Valid state Physical Therapist license required to practice.
- Minimum of two years of direct work experience in a physical therapy capacity, preferably within a home health setting.
- Strong understanding of best practices and clinical practices in physical therapy.
- Excellent interpersonal skills with the ability to foster a cooperative work environment.
- Proficient in using healthcare software and systems for maintaining patient records and documentation.
- Demonstrable ability to develop therapeutic exercise programs and manage patient care plans.
- Exceptional critical thinking and problem-solving skills.
