Duties and Responsibilities

  • Conduct patient assessments including evaluating their condition and needs to develop appropriate therapy treatment plans.
  • Implement specialized techniques of physical therapy, address specific patient needs, and facilitate health improvement.
  • Collaborate with other healthcare professionals to coordinate treatment plans and provide comprehensive patient care.
  • Maintain accurate patient records and document patient care services by entering information into patient medical records.
  • Guide and educate patients and their families about home health techniques, preventative measures, and treatment plans.
  • Manage rehabilitation equipment and assist in training staff and new hires in equipment and therapy techniques.
  • Stay updated with advancements in the field of physical therapy and integrate evidence-based practices into treatment protocols.
  • Assess treatment outcomes and modify treatment plans accordingly to achieve the best possible health improvements.
  • Comply with federal, state, and local certification requirements, ensuring all practices are compliant and up to professional standards.
  • Provide motivational support and guidance to patients to achieve therapy goals and improve overall well-being.

Requirements

  • Master's or Doctorate degree in Physical Therapy from an accredited program.
  • Valid state Physical Therapist license required to practice.
  • Minimum of two years of direct work experience in a physical therapy capacity, preferably within a home health setting.
  • Strong understanding of best practices and clinical practices in physical therapy.
  • Excellent interpersonal skills with the ability to foster a cooperative work environment.
  • Proficient in using healthcare software and systems for maintaining patient records and documentation.
  • Demonstrable ability to develop therapeutic exercise programs and manage patient care plans.
  • Exceptional critical thinking and problem-solving skills.
  • Commitment to ongoing education and professional development in the field of physical therapy.
  • Ability to handle a full caseload and execute responsibilities with minimal supervision.
