VistaRiver is seeking a dedicated and compassionate After Hours Hospice Nurse to join our team in a full-time capacity. This critical role provides specialized care to patients in a hospice setting during after-hours to ensure that their medical needs are met efficiently and effectively. This position is not remote, requiring on-site attendance to provide the highest level of care to our patients during their most critical times. This position requires a compassionate individual who can manage the complexities of patient needs during non-standard hours. The committed professional will work a unique schedule consisting of seven consecutive days followed by seven days off, allowing for concentrated patient care and a balanced personal time-off regime. Duties and Responsibilities
- Provide direct patient care to hospice patients in an after-hours setting, which includes nights, weekends, and holidays.
- Assess patient condition during each visit, ensuring comprehensive documentation of observations and care provided.
- Administer medications and treatments as prescribed by the attending physician and as per the care plan.
- Maintain clear and precise communication with patients, families, and other healthcare professionals regarding patient care and updates.
- Respond to emergency situations with urgency and professionalism, providing expert guidance and interventions as necessary.
- Coordinate with physicians, other nurses, and healthcare professionals in the multi-disciplinary team to enhance patient comfort and prevent complications.
- Document all patient interactions and interventions accurately and comprehensively in the patient's medical record.
- Support family members and caregivers with education on patient care and management of symptoms.
- Ensure that all healthcare regulations, standards, and hospice policies are adhered to during care delivery.
- Maintain patient confidentiality and respects all patients’ rights, providing culturally competent care to a diverse patient population.
Requirements
- Valid RN license in the state of employment, with a minimum of two years of nursing experience required.
- Prior experience in hospice or palliative care preferred, demonstrating a deep understanding of end-of-life care needs and management.
- Availability to work rotational shifts during non-standard hours, including nights, weekends, and holidays.
- Demonstrated ability to work independently in a dynamic and high-pressure environment.
- Strong clinical skills in assessing, planning, implementing, and evaluating nursing care.
- Proven record of reliability and accountability in prior roles.
- Excellent communication and interpersonal skills, capable of managing sensitive situations with empathy and professionalism.
- Ability to maintain a calm and professional demeanor during stressful situations.
- Well-developed organizational skills, with the ability to manage time and prioritize tasks effectively.
- Basic computer skills for effective record-keeping and communication, including proficiency with electronic health records.
