Duties and Responsibilities
- Assess patient’s health problems and needs, develop and implement nursing care plans, and maintain medical records.
- Administer nursing care to ill, injured, convalescent, or disabled patients.
- Advise patients on health maintenance and disease prevention or provide case management.
- Liaise with physicians and other team members to plan and evaluate patient care plans.
- Modify patient treatment plans as indicated by patients' responses and conditions.
- Guide and supervise health care personnel like LPNs and home health aides.
- Manage a caseload of patients, ensuring all patients receive the required care as per the established guidelines.
- Maintain accurate, detailed reports and records of patient conditions and treatments.
- Monitor, record, and report symptoms and changes in patients' conditions in a timely manner.
- Order, interpret, and evaluate diagnostic tests to identify and assess patients' conditions.
- Coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
- Direct and coordinate activities of nurses, assistants, specialists, therapists, and other medical staff.
- Ensure compliance with laws and regulations in health care.
- Participate in educational opportunities to update knowledge and skills.
Requirements
- Must hold a valid RN license in Oregon.
- Preferred experience in home health care settings.
- Expertise in managing a caseload of home health patients.
- Ability to coordinate with LPNs and other members of the home health team.
- Proven ability to provide compassionate and effective care to patients.
- Strong organizational and communication skills.
- Must be able to work efficiently without direct supervision.
- Demonstrated capability in making clinical judgments in a high-paced, dynamic environment.
- Knowledge of computer operations and proficiency in the use of basic word processing, data entry and clinical information systems.
- Adept at handling and resolving challenging clinical situations.
- Commitment to continuing education and professional development.
- Flexible with work hours, able to handle weekend or evening assignments if necessary, and available to respond to emergencies.
- High ethical standards and professionalism, with a commitment to patient privacy.
