Position Summary

Physician’s Choice Home Health is seeking a dedicated and experienced professional to join our team as the Director of Patient Care Services (DPCS). This pivotal role serves as the senior clinical leader, overseeing the planning, coordination, quality, and regulatory compliance of all patient care services delivered in patients' homes. Located within our state-operated facilities, this is a full-time, on-site position that plays a crucial role in ensuring our services meet or exceed standards set by CMS Conditions of Participation, state licensing requirements, and internal quality expectations.

The DPCS is tasked with providing clinical leadership, overseeing care planning and outcomes, ensuring staff competency, and serving as the primary clinical link between physicians, patients, field clinicians, and agency leadership. This non-remote role is essential in maintaining the high standards and excellence associated with Physician’s Choice Home Health.

Key Responsibilities

  • Provide clinical leadership and oversight for all patient care services delivered by nursing, therapy, aide, and interdisciplinary staff.
  • Ensure care is provided according to physician orders, plans of care, and professional standards of practice.
  • Approve and oversee care plans, clinical protocols, and service delivery models.
  • Ensure appropriate clinical supervision, delegation, and scope-of-practice compliance.
  • Ensure full compliance with CMS Conditions of Participation, state regulations, and accrediting agencies (ACHC, CHAP, Joint Commission, etc.).
  • Oversee OASIS accuracy, documentation standards, and clinical record integrity.
  • Lead the agency’s Quality Assurance and Performance Improvement (QAPI) program.
  • Prepare for and lead all state, CMS, and accreditation surveys.
  • Develop and implement corrective action plans when deficiencies are identified.
  • Oversee admissions, discharges, recertifications, and transfers.
  • Ensure every patient receives timely, appropriate, and medically necessary care.
  • Monitor clinical outcomes, hospitalizations, readmissions, and patient satisfaction.
  • Promote continuity of care and coordination across disciplines.
  • Recruit, train, supervise, and evaluate all clinical staff.
  • Ensure clinical competency, orientation, and annual training for all field staff.
  • Conduct performance evaluations and manage disciplinary actions when necessary.
  • Foster a culture of clinical excellence, compassion, accountability, and teamwork.
  • Serve as the primary clinical liaison to ordering physicians, nurse practitioners, hospitals, and referral partners.
  • Ensure timely communication regarding patient condition, plan of care changes, and outcomes.
  • Support business development and referral growth through clinical credibility and responsiveness.
  • Oversee incident reporting, complaints, and grievances.
  • Ensure compliance with infection control, emergency preparedness, and patient safety programs.
  • Conduct investigations and root-cause analyses for adverse events and implement corrective actions.

Required Qualifications

  • Active RN or MD/DO license in the state of Oregon.
  • Minimum 5 years of clinical experience, including at least 2 years in home health leadership.
  • Strong knowledge of home health regulations, CMS CoPs, OASIS, care planning, and interdisciplinary care.
  • Proven leadership, organizational, and communication skills.
