Position Overview: We are currently seeking a skilled and compassionate Registered Nurse (RN) Clinical Supervisor to lead our clinical team in Texas. Job Description Summary:
The Hospice Clinical Supervisor plays a pivotal role in ensuring the effective coordination and delivery of patient care within a home health or hospice setting. Reporting to the Director of Nursing, the Clinical Supervisor oversees the supervision of clinical personnel, ensures compliance with regulatory standards, and facilitates the implementation of quality improvement initiatives.
Job Duties (include, but not limited to):
- Supervise and coordinate the activities of hospice clinical staff, including registered nurses, licensed practical nurses, aides, physical, occupational or speech therapists and other disciplinary team members as assigned: MSW, Chaplain, Bereavement/Volunteer Coordinator and clinical coordinators.
- Conduct initial patient assessments and develop individualized care plans in collaboration with the interdisciplinary team, including physicians, social workers, and therapists, to address patients' goals, preferences, and needs.
- Ensure compliance with regulatory requirements, accreditation standards, and organizational policies and procedures.
- Provide ongoing training, mentorship, and clinical supervision to staff members to enhance their skills and knowledge.
- Perform regular quality assurance audits to monitor the effectiveness of care delivery and identify areas for improvement.
- Collaborate with community resources and agencies to address patients' social determinants of health, such as transportation, housing, and financial assistance, to optimize their well-being.
- Coordinate care transitions between healthcare settings, ensuring continuity of care and adherence to the plan of care during hospitalizations, rehabilitation stays, and hospice admissions.
- Participate in the recruitment, hiring, and orientation of new clinical staff members.
- Act as liaison between patients, families, caregivers, and the healthcare team to facilitate communication and address concerns.
- Adhere to ethical and legal standards of nursing practice, including maintaining patient confidentiality, respecting patients' rights, and advocating for their best interests.
- Monitor and evaluate clinical documentation to ensure accuracy, completeness, and compliance with regulatory requirements.
- Assist in the development and implementation of policies, protocols, and clinical pathways to improve patient care delivery and outcomes.

Position Qualifications:
- Current licensure as a Registered Nurse (RN) in the state of practice preferred.
- Minimum of 2-3 years of clinical experience in home health or hospice care.
- Bachelor's degree in Nursing highly preferred.
- Previous experience in a supervisory or leadership role highly preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to effectively prioritize tasks and manage time in a fast-paced environment.
- Knowledge of Medicare/Medicaid regulations and documentation requirements.
- Commitment to providing compassionate care and advocating for patient rights and dignity.
- Current licensure in the state, CPR certification, and a valid driver’s license.
- Proficiency in electronic health records (EHR) and other relevant software applications.
- Prudent use of office supplies and equipment.
- Fieldwork may expose employees to various conditions such as temperature extremes, physical hazards, and infectious diseases.
- Prudent use of equipment and supplies is essential, with a focus on protecting items from theft and damage.
- Demonstrated knowledge and competency in administrative procedures.
- Ability to communicate effectively with strong assessment, critical thinking, and communication skills, and the ability to collaborate effectively with interdisciplinary teams and engage patients and families in care planning.
- Skills in problem-solving, considering patients' backgrounds, finances, and resources, are essential for delivering optimal care.
- Physical ability to lift and transfer patients, stand for extended periods, and perform essential job functions in various home environments.
- Must meet the physical demands of the job, including lifting patients, carrying supplies, and maintaining manual dexterity.
- Should be prepared to work weekends, holidays, and occasional overtime, with the ability to function independently and cope with the stress of caring for patients with various needs.
- Adherence to the agency's dress code, professionalism in interactions, and compliance with public relation policies are expected.
- They should have reliable transportation to make home visits and assess home safety and maintain their vehicle according to state and organizational requirements.
- Ability to communicate effectively in English.
- Bilingual English/Spanish preferred.
