Job Description Summary:

The Clinical Coordinator provides essential support to clinical administration, patients, families, caregivers, physicians, co-workers, and other healthcare team members by assessing, planning, and evaluating patient situations to ensure quality patient care. The coordinator supervises all patient care activities while reporting to the Home Health Clinical Supervisors.

Job Duties (include, but not limited to):

  • Assist Clinical Administration in assigning staff to new referrals based on patient needs, staff skills, and geographic location. Communicate all relevant information regarding referrals to staff.
  • Maintain accurate field staff schedules, updating them daily based on new assignments and communication from field staff. Submit reports to clinical administration.
  • Manage weekend, holiday, and vacation schedules. Assist Clinical Coordinators in completing weekend and holiday assignments and arrange vacation coverage for field staff.
  • Update hospital/patient tracking logs as needed, using Microsoft Excel.
  • Ensure services provided through contracts and agencies are properly executed.
  • Participate in departmental activities, including staff meetings, in-services, staff development programs, and demonstrate department involvement.
  • Contribute to case conferences by offering suggestions and redirecting discussions when necessary. Perform performance improvement activities as assigned.
  • Maintain good working relationships with co-workers, physicians, hospitals, and other healthcare providers.
  • Act as a liaison in marketing and educating others about agency services, including healthcare professionals and the general public. Identify and offer services to patients in need and at risk.
  • Perform data entry of field staff assignments.
  • Participate in quality improvement activities.
  • Assist with the orientation and training of other staff members.

Position Qualifications:

  • Preferably some college coursework in healthcare administration, nursing, or a related field. 
  • Prior experience in scheduling or administrative roles within the healthcare sector.
  • Knowledge of hospice care practices and regulatory frameworks.
  • Strong organizational and time-management proficiencies.
  • Exceptional communication and interpersonal skills.
  • Proficiency in relevant scheduling software and electronic health record systems.
  • Prudent use of office supplies and equipment.
  • May require occasional weekend, holiday, or overtime work.
  • Compliance with dress code and professional demeanor.
  • Ability to communicate effectively in English.
