Tender Care is hiring a full-time Hospice Medical Social Worker to support patients and families during serious illness and end-of-life care. This is a strong fit for a social worker who wants direct patient impact, a collaborative team, and a local agency where leadership is accessible.

 What you will do

  • Complete psychosocial assessments and support the hospice plan of care.
  • Provide emotional support, crisis intervention, family support, and resource guidance.
  • Participate in IDG meetings and collaborate with nurses, physicians, chaplains, aides, volunteers, and leadership.
  • Assist with advance care planning, placement needs, financial resources, funeral planning resources, Medicare/Medicaid questions, and end-of-life support.
  • Serve as a liaison between patients, families, facilities, hospitals, community agencies, and the hospice team.
  • Support bereavement-related needs and connect families with appropriate resources.
  • Help coordinate and grow the Hospice Volunteer Program.
  • Document timely and maintain patient confidentiality.

 Who we are looking for

  • A professional who is comfortable supporting patients and families from diverse backgrounds.
  • Strong judgment, emotional resilience, cultural sensitivity, and practical problem-solving skills.
  • Bilingual English/Spanish strongly preferred.

 Requirements

  • Master's degree in Social Work required.
  • Current Texas LMSW license required; LCSW candidates welcome.
  • Minimum of one year of social work experience in a healthcare setting required.
  • Hospice experience preferred, but experienced healthcare social workers are encouraged to apply.
  • Valid driver's license, reliable transportation, and a good driving record required.
  • Ability to travel throughout El Paso County to homes, facilities, hospitals, and other care settings as needed.
