Occupational Therapist - Full-time or PRN

Job Description Summary:

The Home Health Occupational Therapist, will play a crucial role in improving the quality of life for individuals in their own homes all while reporting to the Therapy Supervisor. They will be responsible for providing occupational therapy services to patients, assessing their functional abilities, and developing customized treatment plans to help them regain independence and enhance their overall well-being.

 Job Duties (include, but not limited to):

  •  Conduct comprehensive assessments of patients' physical, cognitive, and emotional functioning in their home environment.
  • Evaluate patients' ability to perform activities of daily living (ADLs), mobility, and safety within their homes.
  • Identify home environmental barriers and hazards that may impact patients' independence and safety.
  • Develop individualized treatment plans based on assessment findings, patient goals, and physician recommendations.
  • Establish realistic goals with patients and their families to improve functional abilities and promote independence.
  • Provide recommendations for adaptive equipment and home modifications to support patients' independence and safety.
  • Implement evidence-based occupational therapy interventions to address patients' deficits in ADLs, mobility, and self-care skills.
  • Utilize therapeutic activities, exercises, and modalities to improve patients' strength, coordination, and range of motion.
  • Train patients and caregivers on proper techniques for activities of daily living, adaptive equipment use, and fall prevention strategies.
  • Educate patients and their families on the importance of therapeutic exercises, energy conservation techniques, and safe mobility practices.
  • Provide emotional support and encouragement to patients as they navigate through their rehabilitation journey.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure coordinated care and optimal patient outcomes.
  • Maintain accurate and timely documentation of patient assessments, treatment plans, progress notes, and discharge summaries.
  • Communicate effectively with the healthcare team regarding patients' progress, barriers to treatment, and any changes in their condition.
  • Stay updated on current trends, research, and best practices in home health occupational therapy.
  • Participate in continuing education opportunities, workshops, and professional development activities to enhance clinical skills and knowledge.

 

Working Environment:

  •  Prudent use of office supplies and equipment.
  • Fieldwork may expose employees to various conditions such as temperature extremes, physical hazards, and infectious diseases. Prudent use of equipment and supplies is essential, with a focus on protecting items from theft and damage.
  • Must demonstrate proficiency in procedures outlined in their job description.
  • Effective communication with healthcare professionals, patients, and families is imperative. 
  • Home health occupational therapists may encounter various environments, including private residences, assisted living facilities, and skilled nursing facilities.
  • Flexibility in scheduling to accommodate patients' needs, including evenings and weekends.
  • Physical ability to lift and transfer patients, stand for extended periods, and perform essential job functions in various home environments. 
  • They should have reliable transportation to make home visits and assess home safety. 
  • Adherence to the agency's dress code, professionalism in interactions, and compliance with public relation policies are expected.
  • Ability to communicate effectively in English.

 

Position Qualifications:

 Bachelor's or Master's degree in Occupational Therapy from an accredited program.

  • Previous experience in home health or geriatric care preferred.
  • Strong clinical skills in assessment, treatment planning, and intervention.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage a caseload effectively.
  • Reliable transportation and willingness to travel to patients' homes.
  • Current licensure in the state, CPR certification, and a valid driver’s license.
  • Minimum of one year of clinical experience in acute care, home health, or hospice settings preferred.
  • Management experience not required; responsible for supervising home health aides.
  • Strong observation, communication, problem-solving, and nursing skills.
